Please see attached draft project document (ChildHealthandImmunization) and fill out the highlighted (yellow area) with SMART objectives (three for each technical area); remember these are SMART objectives/goals…. and then complete the worksheet at the end of the document, highlighted (yellow area). The worksheet annex should meet the module requirement for Indicator, (module attached). You will have to design the worksheet to reflect on the SMART objectives, develop target population, indicators and the source of information.
