Overview:
Your community action plan is a professional portfolio that includes a written report, a slide presentation, and a poster or brochure. Review this week’s recommended reading, which was assigned in Week 4 and that provides a nice overview of an action plan. .  All are graded on writing and APA mechanics.   You will include seven to ten references.

You will incorporate into this final plan any classmates’ critiques from the discussion board that you found useful, as well as instructor feedback.

Required Components:

• The written report should include:  900 words 
• • The final case study, with all four sections (Symptoms, Diagnosis, Cure, and Prevention). 
  • An expanded outline of your community action plan.
  • The key community members or partners that you plan on contacting, with an explanation why these individuals or organizations make sense as potential partners. 
  • A realistic timeline for your plan. Discuss the time needed for campaigning, education, funding, building, and implementation. 
• The slide presentation describes the problem in your community and your action plan (five slides minimum, not including required title slide). You may use web-based slide presentation software such as Prezi, for example, or you may use PowerPoint. The audience will be community members or organizations you wish to educate about the public-health issue and your proposed plan.
• The poster or brochure will be 1-2 pages (think of it as a large poster or a double-sided, bi-fold or tri-fold brochure) about the public-health problem and your action plan. The purpose of both the slide presentation and the brochure/poster is to educate community members about the issue. (Keep in mind that in real life you will have limited time to convince people of the gravity of the situation and to come on board with their support, so you want to be persuasive and get to the key points quickly and effectively.)
